How to get a Utah medical cannabis card - EVS application

Get a Utah medical cannabis card in five simple steps.

Find A Utah qualified medical provider

First ask your current medical provider if they are a qualified medical provider (QMP), registered with the Utah department of health (UDOH) and to see if your qualifying medical condition could benefit from medical cannabis treatment.

If you have a qualifying condition but your current provider is not a QMP ask to see if they may refer you to another provider who is a QMP or find a registered provider.

Schedule in-person meeting with QMP to recommend you for the medical cannabis program. Do not start your application online until immediately before or after your appointment with a QMP.

Utah ID Account & EVS Registration

Review the EVS Patient Application Guide before starting your online application.

Patients must create a Utah ID account before starting an application in the electronic verification system (EVS).

Recommendation Issued By QMP

Once you have submitted your online application, your QMP will have to log in online to the EVS and complete their section of your patient application, which includes the recommendation for medicalcannabis treatment.

If you have been diagnosed with a non-qualifying condition, you must upload the Compassionate Use Board Petition form to the EVS.

Cannabis Card Fee & UDOH Review

Once all the step above have been complete you must log back into your EVS profile and pay the medical cannabis card application fee.

  • Patient Card (initial): $15
  • Patient Card (first 90-day renewal): $5
  • Patient Card (six-month renewal): $15

The UDOH will review your application and confirm all requirements have been met. The current application review process timelines are:

  • Adult patients ages 21 and older with qualifying condition - 15 days or less from date the application is submitted.
  • Minor patients younger than 21 and adults without a qualifying condition - 90 days or less from the date the application is submitted. Applications must be reviewed by the Compassionate Use Board
  • Patients younger than age 21 and adult patients without a qualifying condition will receive an email regarding the status of their Compassionate Use Board petition.

Receive Card & Visit Pharmacy

Once the UDOH confirms all requirements have been met your card will be issued and a copy of medical cannabis card will be emailed to you. You may print or save medical cannabis card to your phone.
